Please read closely, as we've recently made changes to the way patients access their medical records.

For Patients with Appointments After 12/6/16

We have transitioned to a new patient portal that allows you to access your medical records, contact your physician, schedule appointments and more.

To access the new patient portal and medical records from appointments after 12/6/16, launch Athena Health.

For Patients Regarding Appointments Prior to 12/6/16

To access any records created during appointments prior to 12/6/16, please use our old patient patient portal, GEMMS

Important Notice
 
Please note access to NextGen will only be available through December 6th, 2016.
 
If you feel you need historical health information from the Gemms portal, you should download and save it as the information will not be available after December 6.
 
After December 6th, you'll need to reach out to your physician's office to get access to any medical records from appointments prior to 12/6/16.
